By enrolling in our Direct Primary Care Membership, you agree to the following terms:

• Membership requires a minimum commitment of six (6) months.

• You have 30 days from the start of your membership to cancel without penalty.

• If you do not cancel within the 30-day period, you are financially responsible for the remaining months of your 6-month commitment.

• Membership fees are non-refundable and will be charged on a monthly basis.

• An invoice will be sent to you on the day you agree to this form, and payment will be due accordingly.

By proceeding with enrollment, you acknowledge and accept these terms.
